We focus on each patient as a whole person and place a strong emphasis on making their life as fulfilling as possible. We support patients to help them to come to terms with living their life differently. Working closely with families and friends is a high priority and we welcome their participation.
Treatment is provided by an experienced team of doctors, nurses, physiotherapists, occupational therapists, speech and language therapists and a dietician. The team work together and use a goal setting, problem-solving approach that is based on evidence and best practice.
All interventions are documented in the patient’s file and/or care plan. Significant decisions about care and treatment are made by patients and families with our support. We provide accurate information of appropriate quantity and depth to enable informed choices to be made.
Where possible, we work with patients in the preparation of their care plan, which details all aspects of their care. If a patient it not able to be involved, we will consult with their close family and friends.
Rehabilitation is a continuous process. We provide a coordinated programme of rehabilitation to enable patients to achieve their goals.
We work with the patient, family and outside agencies to enable successful discharge whether to home or another community setting.
